What is the FD-1164?

The FD-1164 is the fingerprint card published by the FBI for Identity History Summary requests. The form contains spaces for biographical information and fingerprint impressions.

What the FBI says about the paper format

According to the FBI FAQ, the downloadable FD-1164 can be printed on standard white paper. The FBI also notes that a law enforcement agency or private fingerprinting agency may prefer to use fingerprint card stock.

This is an FBI rule about what it accepts. It is not a statement that every foreign authority, channeler or administrative procedure accepts the same format.

A current card is required

The FBI states that a current fingerprint card is required for an Identity History Summary request. A card used for a previous request cannot simply be reused. The FBI also says that submitted fingerprint cards are not returned.

Legibility and completion

The FD-1164 includes official guidance intended to help produce legible impressions. The person making the request remains responsible for checking the current FBI instructions and the requirements of any channeler or authority involved.
